Biliary atresia is the most frequent indication for liver transplantation in childhood. If bile drainage is inadequate after portoenterostomy, portal hypertension develops and liver transplantation is then inevitable. There is an opinion that, under certain circumstances, primary liver transplantation is better than staged surgical procedures. Therefore, liver transplantation is one tool in the life-saving concept for children with biliary atresia.
